Radiesse vs HIFU vs Ultherapy: Restoring Volume vs Lifting Sagging Skin

When patients come in saying their face looks tired or saggy, they often ask for a specific treatment they have read about, usually HIFU, Ultherapy or a filler. But the right choice depends on what is actually causing the change. Is the skin loosening, has the face lost volume, or is it both?

This guide explains how Radiesse, HIFU and Ultherapy differ, and how to tell which one your face needs.

Two different causes of an ageing face

As we age, two things happen at the same time. The skin and the supporting tissue underneath loosen and drop, which shows as jowls, a softer jawline and sagging around the neck. And the face loses volume, from fat pads shrinking and bone gradually changing, which shows as flatter cheeks, hollow temples and deeper folds. Lifting treatments mainly address the first problem. Volume treatments mainly address the second.

HIFU: lifting with focused ultrasound

HIFU delivers focused ultrasound energy to the deeper layers of the skin, including the muscle layer that surgeons tighten in a facelift. This triggers tightening and new collagen without any needles or cuts.

  • Best for: mild to moderate sagging of the jawline, jowls, brows and neck

  • Results: build gradually over 2 to 3 months

  • Downtime: usually none; some redness or tenderness

Ultherapy: lifting with real-time imaging

Ultherapy also uses focused ultrasound, but with built-in imaging that lets the doctor see the tissue layers while treating. This allows energy to be placed precisely at the right depth for each area of the face.

  • Best for: patients who want a precise, single-session lift for sagging skin

  • Results: develop over 2 to 3 months, often maintained with a yearly session

  • Downtime: minimal

Radiesse: volume plus collagen

Radiesse is an injectable made from calcium hydroxylapatite microspheres in a gel. It works in two ways: the gel gives immediate structure and lift where it is placed, and the microspheres then stimulate your own collagen over the following months.

  • Best for: flat cheeks, hollow temples, a softening jawline and hand rejuvenation

  • Results: visible straight away, continuing to improve as collagen builds, and lasting up to around 18 months

  • Downtime: possible mild swelling or bruising for a few days

Because Radiesse supports the face from underneath, restoring volume in the right places can also create a lifting effect, for example in the cheeks and jawline.

Where does Sculptra fit?

Sculptra is another collagen stimulator, made from poly-L-lactic acid. Unlike Radiesse, it gives little visible change at first and builds volume gradually over a series of sessions. At Dr Jane Clinic, we currently use Radiesse as our collagen stimulator because it gives patients both immediate support and long-term collagen benefits. You can read a full comparison in our guide to Sculptra vs Radiesse.

Which one suits you?

  • Mainly sagging, with cheeks that still look full: HIFU or Ultherapy

  • Mainly volume loss, with hollows and flatness: Radiesse

  • Both sagging and volume loss: a combination plan, which is the most common situation after the mid-30s

Combining lifting and volume

Many of our patients get the most natural results from combining treatments, for example Ultherapy or HIFU to tighten the lower face, with Radiesse to restore cheek and temple support. Your doctor will plan the order and spacing of treatments.

Frequently asked questions

Is HIFU or Ultherapy better than filler?

They do different jobs. Ultrasound treatments tighten loose tissue, while Radiesse restores the structure that has been lost. One cannot fully replace the other.

Which lasts longest?

Results from Radiesse can last up to around 18 months. Ultrasound lifting results are commonly maintained with a yearly session. How long results last also depends on your age, skin and lifestyle.

Can I do both?

Yes, and many patients do. A planned combination often gives a more natural, balanced result than relying on a single treatment.
